The proposal form to make an application for the Supported Living Fund is now available from the Ageing Disability and Homecare (ADHC) website.

This Form is known as My Proposal.  You can use the links provided to download the electronic form (PDF) or the form for printing (PDF).

This Proposal form is for consideration in the Round 1 intake – if you are not ready to apply now, please note that there will be another Round later in 2012 and a further Round in 2013.

Proposals Forms are due to ADHC by 5pm Friday 23 March 2012.
