			<content:encoded><![CDATA[<p>The proposal form to make an application for the Supported Living Fund is now available from the <a href="http://www.adhc.nsw.gov.au/individuals/support/somewhere_to_live/slfregistrations" target="_blank">Ageing Disability and Homecare (ADHC) website</a>.</p>
<p>This Form is known as <em>My Proposal</em>.  You can use the links provided to download the <a href="http://www.adhc.nsw.gov.au/__data/assets/file/0004/251878/SLF_Application_eform.pdf" target="_blank">electronic form (PDF)</a> or the <a href="http://www.adhc.nsw.gov.au/__data/assets/file/0010/251884/SLF_Application_print_form.pdf" target="_blank">form for printing (PDF)</a>.</p>
<p>This Proposal form is for consideration in the Round 1 intake &#8211; if you are not ready to apply now, please note that there will be another Round later in 2012 and a further Round in 2013.</p>
<p><strong>Proposals Forms are due to ADHC by 5pm Friday 23 March 2012.</strong></p>

<p>NSW CID and Family Advocacy have gone into partnership to run a series of events for people with disability and their families. Family Advocacy will be running seminars for families in Feb and March 2012.</p>
